Acts as as Case Manager,Coordination support of he Member's utilization of Medic! are benefits.
Educates family member's on healthy promotions, and identifying additional or enhanced services available to maintain or promote better care.
Collaborates with family internist, or specialist to reduce the member's risk of continuous hospitalization visits by conducting telephonic interviews and evaluating current conditions.
Makes recommendations and coordinates necessary treatment, social services, group practice or physician specialists.
Reviews case notes care notes, patients data to review trends, falls, non retention of medication and missed doctor's office visits. After review, determines plan of care, and coordinates solutions involving providers, family member's and other interventions.
Requirements:
RN Licensure (Registered Nurse)
Minimum 5 years prior Case Management experience in home care, hospital, discharge planning, MDS Coordinator or relative Insurance case management experience.
Advanced Medicare In! surance knowledge such as OASIS and CMS guidelines.
Social! Services Coordination, non traditional and community based services background.
Strong interpersonal skills with Patients, Members, Family members, Providers', clinicians, Social Services, Non healthcare constituents.
Prior Disease Management experience
Advanced electronic computer charting and data entry skills.
Advanced CMS and Medicare and Medicaid experience.
