health care including health promotion and disease prevention activities for

CHS patients. Responsible for the

provision o! f quality health care for acute and chronic medical problems;

follows hospitalized patients as appropriate, to assure continuity of

care. Refer patients to appropriate

specialists and consultants for diagnosis and treatment procedures when

indicated. Generally acts as health advocate

for assigned patients.

DUTIES :
1. Provides direct health care to patients, ages 16 and up, presenting for care at CHS with or without appointments. In special circumstances, treating patients who are younger than 16 years of age.

2. Where indicated, obtains complete medical history and physical data on patients. Establishing diagnosis. Reviews patient’s records.

3. Performs clinical services to patients who present with illness or disease.

4. Orders appropriate laboratory and diagnostic procedures.

5. Educates patients regarding health and illness prevention. Recommends community resources to! meet patient and family needs.

6. Instructs patient ! regarding medications and treatment instructions.

7. Adheres to established clinical protocols and procedures.

8. Maintains adequate and legible patient medical records in the established chart.

9. Supervise or participate in patient emergencies as required.

10. Observe all infection control procedures.

11. Fulfills HRSA and health center productivity standards.

12. Follow established health center procedures for patient billing and reimbursement.

13. Participate in quality assurance and other committee activities as assigned.

14. Provision professional supervision of assigned staff.

15. Participate in staff medical education programs and patient health education activities as required.

16. Reports any discrepancies, irregularities, and/or violations involving clinical care, administrative procedure, billing issues, or other Health Center protocols to their immediate supervisor and/or ! the Health Center’s Chief Medical Officer.

17. Maintain professional competence by completing appropriate specialty requirements of CME.

18. Maintain current certifications.

19. Attends required meetings and participates in committees as assigned.

20. Assists with the training of assigned staff.

21. Maintain strictest confidentiality.

22. Participates in professional development activities and maintains professional affiliations.

23. Performs related work as required.

PERFORMANCE REQUIRMENTS:
Knowledge, Skills & Abilities:
Knowledge of professional medical practice and care to give and evaluate patient care. Knowledge of organizational policies, regulations and procedures to administer patient care. Knowledge of medical equipment and instruments to administer patient care. Knowledge of common safety hazards and precautions to establish a safe work environment. Knowledge of report! ing protocols are essential to the proper functioning and operation of ! the Health Center and the responsibility of each provider to comply with them. Skill in applying and modifying the principles, methods and techniques of medicine to provide on-going patient care. Skill in taking medical history, assessing medical conditions and interpreting findings. Skill in preparing and maintaining records, writing reports, and responding to correspondence. Skill in developing and maintaining department quality assurance. Skill in establishing and maintaining effective working relationships with patients, medical staff, and the public. Ability to maintain quality control standards. Ability to react calmly and effectively in emergency situations. Ability to interpret, adapt and apply guidelines and procedures. Ability to communicate clearly.

Education:
1. Graduate of approved medical school.

2. Board certified or Board eligible in Internal Medicine or Family Practice.

3. Licensed to practice medicine in the State of C! onnecticut.

4. Eligible to obtain a federal and state narcotic registration.

Experience:
1. 1-2 years in health care setting preferred.

2. J-1/ H1 accepted.

STATUS: Department of Adult Medicine; Exempt; Full Time; Salary
